The province of Aklan has a new advisory for visitors going to Boracay Island, which was issued by the provincial government on December 1, 2021.

Based on the advisory, visitors planning to go to Boracay Island must click first the link https://aklan.gov.ph/guide-for-tourists/ for AkQuire Online Health Declaration Card (OHDC) QR Code Issuance for tourists going to Boracay Island.

The advisory provides a list of digital copies of documentary requirements that an individual must have in his device before accomplishing the OHDC form application.

The OHDC application is for adults and children including infants (if applicable), according to the advisory.

The needed digital documents are the following: Proof of Identification with Philippine residency /Passport for Foreigners/OFWs/OFs, and Proof of Vaccination Status.

For fully-vaccinated tourists – they must have locally-issued vaccination card/ID with full vaccination status and with verifiable QR code; or locally-issued vaccination card/ID with full vaccination status and with certificate of vaccination from vaccination center with contact details; or vaccination card/ID/certificate issued abroad.

For unvaccinated or partially vaccinated tourists or in absence of the above-mentioned documents, one must submit RT-PCR Negative Result Certificate (saliva,naopharyngeal or oropharyngeal) with date of extraction 72 hours to date of travel; while for children below 12 years old – parents/guardians must accomplish certificate of undertaking travelling with minors template in https://aklan.gov.ph under Tourism.

The tourist must also submit a copy of confirmed booking slip/form from a DOT-accredited Boracay Island accommodation establishment.

For Aklanon tourists residing in Aklan, he/she could use the Certificate of Undertaking for accommodation template in https://aklan.gov.ph under Tourism, in the absence of booking. The incoming tourists must also have roundtrip flight/travel details.

For those coming from Western Visayas and Aklanon tourists residing in Aklan, if they are using private vehicles, they must upload a photo of car plate, however, if they taking public transportation, they must use the Certificate of Undertaking for Travel Details Template in https://aklan.gov.ph under Tourism.

It was emphasized in the advisory that although the OHDC QR Code is autogenerated, the submitted documents will still undergo the usual validation process for completeness and authenticity, thus, the province reiterates the need to upload complete, correct, clear and authentic documents to prevent the OHDC from being invalidated.

The advisory also warned that tourists with invalidated OHDC QR codes may be denied acceptance, boarding or entry by terminals or transport services.AAL/VGV/PIA6 Aklan
